PREPARING FOR YOUR FIRST VISIT

We welcome you as a new patient at Chicago Sports & Chiro in the BASH Sports Academy and we look forward to helping you achieve health and comfort through chiropractic's natural methods. Prior to your appointment, please download each of the new patient forms here. Please have them filled-out and bring them with you to your first appointment. Take your time to gather all of the information about your health, the more complete and accurate it is, the better we will be able to identify any possible issues.  If you have had any recent diagnostic tests performed please bring them with you during your first visit, this way Dr. Castillo can avoid repeating any diagnostic evaluations.

What to Expect at Your First Visit

Patient History

Dr. Castillo will review your forms and discuss your health history. He will ask you questions about your current symptoms and any past care you may have received. All of this information will help Dr. Castillo determine whether chiropractic care is an appropriate treatment for your condition. If he determines that chiropractic care is not the best course of treatment, Dr. Castillo will refer you to the appropriate health care provider.

Physical Exam

We ask that you come dressed in comfortable clothing such as sweat pants and/or shorts, a T-shirt or tank top, because after discussing your health history, Dr. Castillo will perform a series of orthopedic, neurological and spinal examinations to identify any specific problems.  He will also ask you to perform a series of movements to analyze your posture, your movement patterns, and any leg length imbalances. In some cases, the doctor may refer you out to have an x-ray or an MRI before starting your treatment, to rule out anything serious such as a fracture, disc herniation, or an organic anomaly.

Diagnosis + Treatment

Dr. Castillo will determine a diagnosis based on your history, examination and any diagnostic tests. The doctor will present you with an individual treatment plan specifically to address your health problems, taking the time to explain its elements in detail and to answer your questions. Your treatment may involve any combination of services offered at the clinic, including spinal and/or extremity manipulation, soft tissue therapy, functional corrective exercises, electrical stimulation, CLLT or acupuncture. Some patients may even benefit from nutritional and lifestyle counseling. Our goal for every treatment plan is to not only address your current problem, but to improve your health and overall well-being for the long term.
